We require that you fill in the health monitoring questionnaire whenever you test positive for COVID-19, experience symptoms compatible with it or come into close contact with a confirmed case of the disease in the 14 days prior to your attendance at class or work. The filled-in questionnaire will be analyzed by FGV in order to justify your absence from planned in-person activities.
This questionnaire is not intended to perform a clinical diagnosis or to treat diseases or other conditions. It is specifically designed for FGV’s sites, to support measures to prevent the spread of COVID-19.
